Entrust Dental Studio (Dental Laboratory) – GENERAL TERMS AND CONDITIONS
Warranty Terms

* The final restoration’s warranty period from the original date of manufacture is 2 years.
* Diagnostic wax-up / PMMA’s warranty period from the original date of manufacture is 6 months.
In particular, PMMA is due to the higher incidence of damage or ill-fit caused by intermittent wear and/or dental, skeletal, or muscular forces or change therefore most dental laboratories provide a warranty for 90 days. But our PMMA will provide more like a final shape, color, and strength for long-term use. Entrust Dental Studio LLC [Laboratory] will provide 180 days for the warranty, proudly.
* Implant part & Custom Abutment’s warranty according to Implant manufacturer warranty. We will provide warranty information when you choose your preferred manufacturer. Check more detail about " Implant Warranty "
Remake Policy & Warranty

ENTRUST DENTAL STUDIO LLC [Laboratory] GUARANTEE YOUR COMPLETE SATISFACTION with the workmanship and materials of the Restoration you’ve ordered. If the restoration is not acceptable at the time of invoice or at the time of insertion, barring service warranty conditions described below and provided prior restoration history does not reveal an unwarranted order, we shall adjust, repair, or replace the restoration at no charge, preserving the full warranty period. Warranty conditions are listed below, and the warranty period begins on the date of manufacture. Simply return the restoration together with an explanation of the problem and your request for a restoration adjustment, repair, or remake.

Remake Policy
​
BEFORE PERIOD WARRANTY,
(1) covers your restoration NOT in use (before insert or while tried in or temporarily cemented/removable), against defects in workmanship and materials. Simply return the restoration together with an explanation of the problem and your request for a restoration to remake and a new impression, pick up an impression, or send a new scan file if necessary. But, if the final restoration or during the try-in service, if the dentist or clinical staff (including the patient) sides made a fractured clinical instrument or lost the restoration so cannot be able to repairable, our main service costs will be covered, but we will be charged for the base material of the restoration. For this case, the warranty will be the remainder of the warranty period from the original date of manufacture remains intact.

(2) covers your restoration in use (permanently cemented), against defects in workmanship and materials. In this event, the remainder of the warranty period from the original date of manufacture remains intact. Simply return the restoration together with an explanation of the problem and your request for a restoration to remake and a new impression, or pick up an impression or new scan if necessary. We will charge only for the base material of restoration. If unsatisfactory material performance is experienced within the service warranty period or the restoration comes off, the patient swallowed the restoration, so we have to remake the restoration, excluding clinical costs, which are covered, but we will be charged for the base material of the restoration.
AFTER PERIOD WARRANTY, if restoration (permanently cemented) is used or has never been inserted in but time was over, the restoration is an unexpected situation - came off and lost or swallowed, fractured, or any damage of the restoration in use, against defects in workmanship and materials, for all cases will not be covered by our warranty. Even if unsatisfactory material performance is experienced after the service warranty period, all of the remake cases of the restoration have to be 100% charged. And the remake case's warranty will be restarted from the remade date, like a brand-new case. We strongly recommend restarting all processes like a new case. If we find out reused the prior materials after the expired warranty, we cannot guarantee all the remake risks, and depending on the cases, the warranty may NOT cover properly.

WHAT CASE WILL NOT BE COVERED BY THE WARRANTY,
(1) After the warranty expired.
(2) The Dental Practice (Office/Dentist) provided instruction or prescription that was wrong, missing, or not enough information, distorted, or a contaminated source, defective oral scanned file, ignored our notice of risk and let us work with it; therefore, all the materials and sources were not useful, so we had to remake them. However, if it can be minor adjusted or repaired, we won't charge any cost before the period of warranty.

(3) If the laboratory notifies the office or the doctor the case cannot be quality guaranteed and risks failure during the lab process or office treatment procedure and you receive a confirmation response from them asking you to proceed with the current situation without any corrections, if the remake or plenty of material need to add to adjustment cases sources have come from the office or the doctor or patients, the laboratory may be asked to pay the full amount or a certain percentage amount depending on the situation.
(4) If you want a remake of the restoration in a situation other than a simple change of mind, or against defects in workmanship and materials, or technical reasons.
(5) After delivery, the completed case package is damaged, lost, or stolen. It is proved by UPS/FedEx/ USPS Tracking source or our delivery driver signed on the delivered case sign document or proof photo shot after dropping confirmed by a member of the dental office/practice.
(6) The patient is ill-fitting due to an accident, patient non-compliance, negligence, abuse, supporting bone, tooth or tissue failure, dental change, dental or muscular parafunctional habit, improper clinical protocols, or improper oral hygiene. Unwarranted orders include cases requiring clinically requested lab alterations to original case materials, as well as clinically overruled laboratory requests for new materials, products, or process changes. Further, service warranties do not include or apply to devices that have been previously non-warranted in the device history record, or have failed due to misdiagnosis or incorrect choice of device. Warranties do not include nor apply to work performed by another laboratory, cash refunds, temporary replacements, costs incurred for removal or reinsertion, or costs incurred by another laboratory. All guarantees and warranties exclude sales to other laboratories. This warranty is instead of all other warranties, either expressed or implied, and may not be modified by any agent, employee, or representative of the laboratory.

(7) [ Rush Case ] If someone expected the case to get back rushed than our standard working term, we can not guarantee the high-end quality according to client expectations. We inform you that if we speed up the case to finish, we do not have enough time to study, resolve problems, or quality check for it. We also share the burden of working time with all other existing cases. For such a reason, we should charge 50% or the full original amount if the case needs to be remade. We softly ask you to understand these points for our relations.

Entrust Dental Studio LLC (Laboratory) reserves the right to make modifications and updates to the warranty at any time.

*IMPLANT PART & CUSTOM ABUTMENT'S WARRANTY
​
Depends on the choice of your preferred manufacturer's warranty policy. However, remaking or reordering the stock abutment/custom abutment with the original manufacturer's warranty mostly covers when they figure it is their mistake only. The original manufacturer will be charged the full amount of the order and the shipping fee for every order submitted.

(1) Implant part & Custom Abutment, we ordered the wrong part or designed and lost the parts in the Entrust Dental Studio LLC [Laboratory], we will respond to pay on our own. It will not charge on the dental practice.
(2) Implant part & Custom Abutment, the member of the dental practice requested the wrong manufactured parts or size information took an impression or oral scanned when the implant coping or scan body miss-placed direction or did not fit in properly, against defects in products and materials, during the try-in service if the dentist or clinical staff side made a damaged on the abutment or screw hex by clinical instrument, came off and lost or swallowed of all kind of abutments and screws, will be a charge to the dental practice, every each event appears.​
(3) If it needs to be placed over the 4 units of implant parts & custom abutments for the case, Entrust Dental Studio LLC [Laboratory] will ask you to prepay 20% of the total final bill. The case may be on hold until the prepaid payment is received.
